Rehabilitation and Support Services Geelong
We provide the following flexible community based rehabilitation and support programs for people who are experiencing problems as a result of a mental illness.
Programs include:

Day Rehabilitation Program
Pathways has researched and developed a structured pathway of care to ensure the best possible outcomes for participants. The Day Rehabilitation program provides:
- Encouragement to be actively involved in determining a personal pathway to wellness
- Support to identify whole of life issues that are having an impact on personal wellness
- A Key Worker to identify strengths and provide individual support to reach personal goals
- Coordinated planning with other people who also provide care
- Support to find and maintain affordable housing
- Education about mental illness and how to manage symptoms
- Purposeful and goal orientated group activities
- Social activities to build support networks and maintain wellness
- Support to access services to meet any other needs
- Access to extra specialist support to develop personal strategies to address difficulties
- Carer information and support
- Access to recreation, education, volunteer or paid work in the community
- Access to work preparation and supported work programs.
Prevention and Recovery Care (PARC)
PARC, a joint program between Pathways & Barwon Health, provides both day package and residential support to clients who are considered to be at risk of experiencing an acute episode or relapse of their mental illness without increased support. PARC can also provide care to people who have recently experienced an acute episode or relapse in their mental illness as an alternative environment for continuing their recovery. Referrals to PARC are accepted from Barwon Health Mental Health Team Case Managers.
The Social Connections Program is for older people who are particularly disadvantaged by their long term mental illness and social isolation.
This program is for people who:
- are socially isolated
- have long term complex issues relating to their mental illness
- are physically independent – this is essential due to the type of activities being offered.
This program has been designed to offer people a range of community based social and recreational activities that provide opportunities to socialise and develop personal skills that in turn, encourage independence. Some of the activities on offer are bowling, volleyball, billiards, walking, swimming, art, music, day trips, tours and special interest sessions.
Youth Connections Program
This intensive outreach program has been designed for isolated, 16 – 25 year olds who due to illness are not actively participating in community activities and are unable to connect with support services that would normally assist them. Support is delivered on a one-to-one basis with the aim of engaging the young person with the community or support services.
In-House Support Program
The In-House Support Program is an accommodation and outreach support service for people who are significantly disadvantaged by mental health issues.
It is a program for people who have experienced loss of skills as a result of their mental illness.
The In-House Support Program provides individual rehabilitation support to people in a variety of settings, for example:
- Office of Housing accommodation
- Private rental properties
- Group homes managed by Pathways
- The persons' own home
The aim of this program is to support people to learn or relearn daily living and social skills and to reach their potential for independent living in the community. Ongoing lifestyle education is also provided to maximise every opportunity for participants to achieve their personal goals.
In addition people are given support to access a wide range of services in the community to ensure they are socially connected and receiving any other services they may need.
Homeless Outreach Psychiatric Service (HOPS) Program
Pathways, in partnership with Barwon Health, provides a quality clinical and psychosocial assessment and treatment service, secondary consultation to the homeless sector and formal education sessions to providers of both homeless and mental health services. Short term emergency housing and support for crisis situations are available for homeless people referred through the Mental Health Service System.
Eating Disorders (ATOLL) Program
The ATOLL Program is an intensive outreach program for people with disordered eating. Pathways accepts referrals for this program from the staff of the Barwon Health Eating Disorders Service Barwon Region.
Psychological Counselling
Pathways' psychologist can assist and develop strategies to address identified difficulties and manage symptoms.
DBT Program
The DBT skills training program at Pathways Day Rehabilitation is run in partnership with Barwon Health. The program is primarily designed for people with borderline personality disorders. Assessment for this disorder will be needed before referral to the program.
Puzzle Program
This program is part of Pathways Day Rehabilitation Program and is run in partnership with Barwon Health's Jigsaw service and Clockwork. It is for young people aged 16 - 25 who are currently being seen by a worker at a Barwon Health Early Intervention Service, Jigsaw, Clockwork or Pathways.
Connecting Friends
This program is for people with long term mental illness who would benefit from getting out about about and socialising with others.
